During the Visit

The following are examples of the types of interventions that may occur during a patient's visit or encounter with a facility, pharmacy or testing site.

  • Facility has identified data use needs for clinician, manager, and administrative staff.

  • Health facility uses a standardized tool/method to document client follow-up outcome. Health facility has adopted standard operating procedure to track patient transfers, identify unique client, and verify data quality.

  • Data analysis and use are reinforced through supportive supervision.

  • Health facility takes action on reported indicators only when feedback is received from external groups.

  • An external party (district health office staff, implementing partner staff) provides supportive supervision on data use to health facility staff on a regular basis.

◎Health facility takes action based on review of data in the system, based on changes or other information available.
