Point care –queues (to track patient care and to know whether patients are missing appointment or missing picking up medications etcetera.)

Developed a Mobile app to outreach patients who missed patients and provide community-based health service delivery.

This app helps to increase the outreach by tracking the people who lost appointments.

Factors to Scale

Patient flags are used for analysis of patient data. It is same as traffic light analogy.

Green flag-patient is attending the visits.

Orange flag- patient has 3 more chances (like a danger zone).

Red flag- Patient lost complete follow up
