Intervention Description
Identifying patients who missed appointments using Tracking Retention and Client Enrollment (TRACE) process, and a mobile app to conduct outreach to bring the patients back to care.

Two week and six weeks TRACE reports are generated for missed appointments. In two weeks report, CHWs alert HIV and NCD clients on missed appointment. In six week report, IC3 (Integrated chronic care clinic) TRACE team follows the HIV and NCD clients. | |
