Implementation Considerations

Two week and six weeks TRACE reports are generated for missed appointments.

In two weeks report, CHWs alert HIV and NCD clients on missed appointment.

In six week report, IC3 (Integrated chronic care clinic) TRACE team follows the HIV and NCD clients.

Governance Considerations

Consent to communicate by phone may be needed.
