Project HOPE and Dimagi presented Unified Data System to standardize data capture and case management system for all PEPFAR and USAID funded community HIV activities. The system allows Community Engagement Facilitators (CEFs) to document and share tracing efforts and outcomes.

Evidence | Figures explain that nearly 15084 LTFU line list was received. Out of which, 9907 were traced and located, and 8082 LTFUs were reengaged back to care. Among 15084 LTFU line list, 5177 were unable to trace and locate. Evidences suggest that forgetfulness (patient related), side effects (medication related), long waiting times (health system related) and lack of transport money (socio economic related) are the main reasons for LTFUs among individuals. |
